En France l'usage du cannabis est très souvent associé à celui du tabac. Le pouvoir addictif du cannabis est relativement faible, celui du tabac important. L'usage régulier de tabac et de cannabis est à l'origine du renforcement de la dépendance à chaque substance et rend l'arrêt de ces consommations souvent difficile en même temps qu'il favorise la survenue de complications somatiques et psychiques. Les praticiens doivent repérer ce double usage, conseiller l'arrêt, proposer leur aide à cette fin, faire le bilan des consommations et de leurs soubassements. La prise en charge associe un soutien psychothérapeutique ; la substitution nicotinique permet de limiter le syndrome de sevrage et le craving lors de l'arrêt et accroît l'adhésion des patients au suivi thérapeutique. La mobilisation des praticiens dans la prise en charge des patients et celle des chercheurs dans la mise au point de pharmacothérapies efficaces dans l'arrêt du cannabis s'imposent. ENGLISH: In France, the use of cannabis is very often associated with that of tobacco. The addictive power of cannabis is relatively low, that of tobacco is high. The regular use of tobacco and cannabis is at the origin of the reinforcement of the dependence on each substance and makes it often difficult to stop such consumption, at the same time as it favours the occurrence of somatic and psychological complications. Practitioners must identify this dual use, advise stopping, offer their help after taking stock of consumption and their bases. The care combines psychotherapeutic support and if no medication has proven efficacy to aid stopping cannabis, nicotine replacement therapy makes possible to limit withdrawal syndrome, and craving; it increases patients' adherence to therapeutic monitoring. The mobilization of practitioners in patients care and of researchers in the development of effective pharmacotherapies in stopping cannabis appears essential. |
