This chapter describes how HLS men, most of whom have never been in treatment, view the drug treatment system, what drug treatment means to them, and what inducements would be needed for them to enter and stay in treatment. It concludes with questions about current treatment practices and suggests factors that should be considered in planning treatment strategies in the future.
Although methadone is not the only treatment modality, it is the focus of this discussion because it is the predominant modality in the urban areas where HLS men live. In answering the question, "What do you think about the current treatment programs for heroin addicts?" most HLS men (72 per-cent) referred specifically to methadone maintenance. Indeed, a majority (68 percent) of the Black clients in federally supported treatment received methadone detoxification or maintenance, compared to only 20 percent who received outpatient drug-free services and 12 percent who received residential care (NIDA 1981). A brief history of methadone maintenance in the United States is presented as a backdrop against which comments by the HLS men can be understood. [Extract] |
