FRANÇAIS : Larticle commence par une brève description de lépidémiologie de la dépendance à lhéroïne aux Pays-Bas et par une revue sommaire du système actuel de soin mettant laccent sur la maintenance à la méthadone par voie orale. Vient ensuite un bref historique du débat sur la prescription dhéroïne aux patients dépendants. Cet historique révèle quaux Pays-Bas on considère les alternatives à la méthadone comme essentielles depuis le début des années 1980. Des expérimentations antérieures de maintenance à la morphine en intraveineuse, à la méthadone en intraveineuse et au dextromoramide (Palfium ®) par voie orale ne se sont pas révélées aussi efficaces quespéré, et nont donc pas été généralisées. Enfin, suite à des expérimentations récentes de prescription médicale dhéroïne en Suisse, une étude scientifique sur la co-prescription médicale dhéroïne a été lancée aux Pays-Bas. Larticle décrit de manière détaillée lessai clinique randomisé de lefficacité de la co-prescription dhéroïne qui est actuellement en cours aux Pays-Bas. En conclusion, on examine brièvement les conséquences probables de différents résultats de cette étude pour les options de traitement à venir. ENGLISH : This paper begins with a brief description of the epidemiology of heroin addiction in the Netherlands and an overview of the current treatment system, with special emphasis on oral methadone maintenance. It is followed by a short history of the debate over the prescription of heroin to addicted patients. This history reveals that alternatives to methadone have been viewed as essential in the Netherlands since the early 1980s. Earlier experiments with intravenous morphine, intravenous methadone and oral dextromoramide (Palfium) were not as successful as had been hoped and precluded their introduction in daily practice. Finally, recent experiences in Switzerland with the medical prescription of heroin facilitated the decision to initiate a scientific study of the medical co-prescription of heroin in the Netherlands. The paper provides a detailed description of the randomized clinical trial regarding the effectiveness of co-prescribed heroin that is now underway in the Netherlands. It concludes with a brief discussion of the likely consequences for future treatment options of different study outcomes. (Author's abstract.) |
