FRANÇAIS : Dans le but d'identifier les individus s'adaptant difficilement aux changements thérapeutiques et de suggérer des modalités de traitement réduisant ce problème, cet article examine les réactions de 177 patients sous maintenance méthadone face à un changement de présentation de la méthadone, et les caractéristiques de ceux qui s'adaptent mal et des autres. Trois types de données sont analysés : sociodémographie, évolution des doses de méthadone et facteurs de prédiction permettant de différencier les patients. Trois variables sociodémographiques paraissent prédire des patients intolérants aux changements : le sexe, l'histoire des traitements antérieurs et l'abus de méthadone. Des suggestions sont proposées pour minimiser cette intolérance et son l'impact sur le personnel soignant (réaction de confusion, d'anxiété et d'incertitude). ENGLISH : By identifying individuals, in methadone maintenance, who have substantial difficulty adjusting to changes in methadone formulation and shifts, 177 patients who were receiving methadone maintenance in a program that changed methadone dispensing formulations were examined. This study revealed significant differences between change tolerant and intolerant patients groups . These patients were distinguished by personal characteristics prior to the formulation change and methadone dose level shifts after the formulation change. Change-intolerant patients experienced formulation-related narcotic withdrawal whereas change-tolerant patients adapted to the new medication format. Gender, treatment history, and methadone abuse were three sociodemographic variables that significantly predicted patients who were intolerant to a methadone formulation change. These findings suggest the need for more research, including plasma and metabolic studies, to clarify the potential biological effects of changes in methadone formulations.
