L'intersecteur d'alcoologie permettant la proximité de l'offre de soins, des suivis de longue durée et le travail en réseau avec les partenaires médico-sociaux, se donne les moyens du travail de réadaptation à la vie sans alcool des patients sevrés et ne favorise théoriquement pas l'orientation vers des centres de postcure. Toutefois, au cours des six dernières années, nous avons orienté 30 patients vers des institutions de postcure. En réévaluant à distance les indications de ces orientations, nous avons pu préciser deux objectifs prioritaires pour ce type de séjour : - permettre à certains patients qui ont besoin d'être protégés plus longtemps des risques d'une réalcoolisation rapide de bénéficier de séjours de longue durée (supérieurs à deux mois) dans un contexte thérapeutique adapté ; - la mise à distance (kilométrique) des tiers habituels pour des patients se trouvant dans une profonde crise familiale ou professionnelle. Ces deux objectifs correspondent à des prestations que ne peut offrir un Intersecteur d'Alcoologie. Il existe là une véritable complémentarité entre deux types d'institutions thérapeutiques correspondant pourtant à des concepts médico-économiques différents. (Résumé d'auteur) ENGLISH : By allowing proximity of health care, long-term follow-up and networking with medical and social partners, the multidisci plinary alcohology unit facilitates the post-withdrawal patient's readaptation to life without alcohol and theoretically does not require referral to a post-withdrawal centre. However, over the last six years, we have referred 30 patients to post withdrawal institutions. Two main objectives for this type of treatment were defined by re-evaluating the long-term indi cations for these referrals: - to allow some patients, who need longer protection from the risks of rapid alcoholic relapse, to benefit from longer stays (more than two months) in an adapted therapeutic setting; - physical separation from the patient's usual surroundings in the case of a major family or work crisis. These two objectives correspond to elements that cannot be provided by a multidisciplinary alcohology unit, demonstra ting the real complementarity between two types of therapeutic institution, corresponding to different medico-economic concepts.
