L'objectif de cette étude était d'examiner une nouvelle méthode permettant de désintoxiquer les patients dépendants aux opioïdes. Le butorphanol est un opioïde ayant des propriétés à la fois agonistes et antagonistes, et qui est vendu sous la forme d'un pulvérisateur nasal. Non dilué, il provoquera des symptômes physiques de sevrage chez des personnes dépendantes aux opioïdes. 40 patients dépendants aux opioïdes ont été désintoxiqués en utilisant un vaporisateur de butorphanol dilué. La concentration initiale était de 40% et a été diminuée graduellement. Cette technique a été très efficace pour arriver à maintenir en traitement, et pour une période plus longue, ce groupe de patients difficiles. Ce temps gagné augmente la probabilité de pouvoir envoyer ces patients dans les services post-cure. Il y avait un sous-groupe de patients qui souffraient de douleurs chroniques et qui étaient dépendants aux opioïdes, mais qui pouvaient relativement bien supporter la douleur grâce au spray de butorphanol à une concentration de 10 à 20%. Cette approche unique et inhabituelle mérite de plus amples discussions et études scientifiques. ENGLISH: The purpose of the study was to investigate a new method for detoxifying opioid-dependent patients. Butorphanol is an opiod with mixed agonist-antagonist properties, and is marketed as a nasal spray. Undiluted, it will cause significant physical withdrawal symptoms in a population dependent on opioids. Forty patients dependent on opioids were detoxified using dilute butorphanol spray. The initial concentration was 40% and gradually reduced. This technique was highly successful in keeping this difficult group of patients engaged in the treatment process for a longer period. This time increased the probability of getting these patients involved with postdetoxification services. There was a subset of patients who had chronic pain and were opioid dependent, who had adequate control of pain with butorphanol spray at 10 to 20% concentration. This unique and unusual approach is worthy of open discussion and further scientific studies. (Author' s abstract) |
