FRANÇAIS : Un réseau local de professionnels de santé, travaillant en étroite collaboration avec un médecin conseil de l'assurance maladie, s'est constitué autour de la prise en charge de patients toxicomanes substitués par buprénorphine-haut-dosage (Subutex°). Parmi eux, les 8 médecins libéraux ont évalué leurs pratiques sur un échantillon de leurs patients (n=85) en analysant l'évolution de certains critères de juin 1997 à juin 1998. Globalement, la situation sociale et l'état médical des patients sont améliorés ou stabilisés. Les praticiens veilleront à remédier aux carences en matière de soins dentaires qu'ils ont constatées dans un grand nombre de cas. Cette démarche locale de santé publique démontre le rôle important que peuvent jouer des médecins généralistes impliqués et informés dans la prévention et les soins de proximité des patients toxicomanes traités par substitution. ENGLISH : A local network of health-care professionals, in collaboration with a physician from the medical service of French national health insurance, was constituted in order to study the care given to drug-addicted patients receiving high-dose buprenorphine drug maintenance (Subutex°). The eight practicing physicians involved in the program evaluated their practices in samples of their own patients (n = 85) by analyzing a number of dif- ferent criteria from June 1997 to June 1998. Overall, the social environment and general health of the patients studied improved or remained unchanged. Having observed a large number of dental problems in these patients, the physicians ordered appropriate follow-up dental care. This local initiative in public health demonstrates the important role which can be played by involved general practitioners properly informed in the pre- vention and care required by drug-addicts treated with maintenance therapy.
