FRANÇAIS : Une tomographie d'émission de positrons (PET) a été réalisée chez 10 toxicomanes injecteurs d'opiacés, exposés à 6 séquences de stimulis alternatifs, par des vidéos évoquant les drogues ou non, et ceci à 2 reprises. Après le deuxième scanner, on administre à chaque personne de l'héroïne ou un placebo, selon un procédé en double aveugle. Ceci permet des investigations de l'activité cérébrale selon les états subjectifs relatés, en absence ou en présence d'héroïne. On observe que les témoignages de "besoin de produit" sont fortement corrélés à une augmentation du flux sanguin (rCBF) dans les régions cibles du cortex inférieur frontal et orbitofrontal du système mésolimbique dopaminergique impliqué dans le système de conditionnement et de récompense. L'appétence pour le produit est aussi associée à une augmentation significative du rCBF dans le pre-cuneus droit, une zone associée à la mémoire temporaire, et à l'insula gauche, zone impliquée dans les composantes émotionnelles du stimulus. Le fait de se sentir "planer" est lié à l'activité du rCBF dans l'hippocampe, zone en rapport avec le renforcement des acquitions liées aux stimuli. ENGLISH : Ten male opiate addicts, who were current heroin injectors, underwent positron emission tomographic (PET) scanning during exposure to a sequence of six alternating drug related and neutral video cues, on two occasions. After the second scan, each subject received heroin or placebo using a randomised single-blind procedure. This design allowed the investigation of patterns of brain activity during a range of self-reported cue evoked emotional states, both in the presence and absence of heroin. Self-reports of 'urge to use' correlated strongly with increased regional blood flow (rCBF) in the inferior frontal and orbitofrontal cortex target regions of the mesolimbic dopaminergic system, implicated in conditioning and reward. "Urge to use" was also associated with highly significant increased rCBF in the right pre-cuneus, an area associated with episodic memory retrieval, and in the left insula. implicated in the processing of the emotional components of stimuli. Self-reports of feeling "high" correlated with rCBF activation in the hippocampus, an area relevant to the acquisition of stimulus-associated reinforcement. (Author's abstract.) |
