FRANÇAIS : Différentes lignes d'études, regroupées autour d'un noyau commun, ont démontré l'existence de multiples facteurs en relation avec les conduites à risque de transmission du VIH. A travers une revue bibliographique, cette étude porte sur les principaux facteurs bio-psycho-sociaux qui sont étroitements liés à l'apparition et au maintien des pratiques à risque. Certains de ces facteurs semblent échapper aux possibilités d'interventions dont dispose le dispositif de prise en charge. Cependant il existe d'autres facteurs, essentiellement de type psychologique (moment de la notification, réalisation du test positif au VIH, sentiment de vulnérabilité, "distorsion cognitive") sur lesquels on peut agir pour diminuer ces conduites à risque. Des réflexions et des recommandations sont données sur ce sujet. Les auteurs soulignent l'importance du traitement de certaines formes d'addiction, en particulier, la polyconsommation d'opiacés et de cocaïne, qui influence significativement le maintien des conduites à risque. La détection et le traitement des troubles de la personnalité, principalement des troubles limites et des personnalités antisociales sont recommandés car ils sont fortement corrélés aux conduites à risque. ENGLISH : Different lines of study, joined by a common link, have demonstrated the existence of multiple factors related to risk behaviour for the transmission of HIV. Through meansof a bibliographical revision, we hope to offer a holistic vision of the main bio-psycho- social factors which show the he closest links with the appearance and the sustenance of these high risk practices. Some of these practices would escape the possibility of coming under the supervision of aid resources. However, other factors do exist, essencially of a psychological nature (the moment of notification, the carrying out of the HIV serological test, the feeling of invulnerability, "cognoscitive distorsion", etc.), about which we can act, in order to diminish the foresaid forms of risk behaviour. We hope, therefore, to offer some reflections and recommendations for what to do about them. We must point out the importance of the treatment of some drug addictions, principally the double addiction to opiates and cocaine, because of its important influence over the sustenance of risk behaviour. Lastly, we recommend the detection and treatment of personality disorders, mainly those of extreme and antisocial disorders, which are the ones that hold the closest relationship with risk behaviour.
