Le thème de la réinsertion professionnelle du sujet alcoolodépendant sevré recouvre à la fois le maintien dans l'entreprise et l'insertion dans l'emploi du patient en rupture professionnelle. Le récent arrêt du 15 novembre 2000 de la Cour de Cassation pourrait à l'avenir augmenter le nombre possible de licenciements dus à l'alcool. En France, chaque salarié a un médecin du travail qui, dans le respect du secret médical, est habilité à faire des propositions d'aménagement de poste de travail ou de reclassement. La visite de pré-reprise a une place particulièrement importante. Il y a une grande variabilité dans la résolution du problème alcool dans les entreprises françaises. Ces réflexions amènent les recommandations suivantes : - Le médecin du travail doit pouvoir parler alcool, donner le conseil minimal et orienter l'alcoolodépendant vers une démarche de soins. - L'équipe soignante doit conseiller au sujet de prendre contact avec le médecin du travail. - Conseiller le plus précocement possible à ce sujet de faire une demande de reconnaissance de travailleur handicapé qui donne accès à un dispositif spécifique. - Fournir à ces structures spécialisées (organismes de placement et de réinsertion) un appui technique et une formation en alcoologie afin de mieux identifier le problème alcool. (Résumé d'auteur) ENGLISH : The topic of occupational rehabilitation of abstinent alcohol-dependent patients includes maintenance of the patient's pre- vious job and finding a new job when the patient was previously unemployed. The recent French Court of Appeal decision on 15 November 2000 could possibly increase, in the future, the number of dismissals related to alcohol. In France, each employee is followed by an occupational health physician, who, while respecting medical secrecy, is authorized to propose work station adjustments or job reclassification. The visit prior to return to work is particularly important. There is a marked variability concerning resolution of the alcohol problem in French companies. These findings lead to the following recommendations: - The occupational health physician must be able to talk about alcohol, provide a minimum of advice, and refer the alcohol-dependent subject for treatment. - The healthcare team must advise the patient to contact the occupational health physician. - This patient should be advised, as rapidly as possible, to apply for disabled employee status, which gives access to a specific structure. - Provide these specialized structures (placement and rehabilitation organizations) with technical support and training in alcohology in order to more clearly identify the alcohol problem.
