FRANÇAIS : Une consommation modérée d'alcool, en particulier de vin en France, est associée à une remarquable protection contre les maladies cardiovasculaires. Ceci malgré un effet, à l'évidence, néfaste sur la majorité des facteurs de risque traditionnels de l'infarctus du myocarde comme les lipides sanguins, la pression artérielle, l'acide urique, le poids (et l'index corporel), le glucose, etc. Ce paradoxe indique que nous ne comprenons pas pourquoi (ou comment) l'alcool à doses modérées protège. Notre programme de recherche a précisément pour but d'explorer la possibililité que la protection induite par la consommation d'alcool s'exerce, au moins partiellement, par l'induction d'une sorte "d'immunité myocardique" contre l'ischemie ; comme si la consommation modérée d'alcool entraînait des modifications métaboliques myocardiques rendant le coeur résistant au manque d'oxygène. Ce type d'approche n'est possible que sur l'animal d'expérience évidemment. (Extrait du document.) ENGLISH : Moderate consumption of ethanol, in particular of wine in France, is associated with a significant protection against the risk of cardiovascular diseases (see for instance: de Lorgeril et al. Wine drinking and risk of cardiovascular complications after recent acute myocardial infarction. Circulation 2002, vol. 106, p. 1465). This effect occurs despite a deleterious effect on most of the conventional risk factors of coronary heart disease (CHD). Further studies are obviously required to try to explain the mechanisms by which a moderate consumption of alcohol protects against CHD. Our research programme is aimed at exploring whether the alcohol effect participates, at least partially, in the induction of a kind of " myocardial immunity " against ischemia; as if some metabolic alteration in the myocardium might render the ischemic myocardium resistant to a relative lack of oxygen, We are using the experimental approach, specifically in the rat (Langendorf model). Our preliminary data are extremely encouraging suggesting that the rats that drink alcohol (about 10 and 20% of total energy intake) are actually more resistant to ischemia than the rats that only drink water. That resistance, contrary to our expectation, seems to be independent of an effect on protein kinase C translocation. (Author's abstract.) |
