L'entretien motivationnel (EM) est une méthode de communication centrée sur le patient, peu développée jusqu'à présent en France. Son intérêt est maintenant bien démontré dans toutes les situations thérapeutiques où ambivalence et motivation sont au centre d'un processus de changement. Les addictions et l'éducation thérapeutique sont particulièrement concernées ici. À la philosophie et aux principes de l'EM répondent une série de techniques bien précises pour aider à l'exploration et à la résolution de l'ambivalence face au changement, considérée comme normale dans l'approche motivationnelle. Cet article, après avoir défini les principes et les stratégies, décrit quelques-unes des techniques principales et cherche à positionner clairement l'EM par rapport aux autres approches thérapeutiques s'intéressant aux changements de comportement. Enfin il précise les modes d'intégration de l'EM dans une stratégie thérapeutique. La souplesse d'utilisation de l'EM en association avec d'autres modalités thérapeutiques, l'évolution radicale de la relation patient-soignant qu'il sous-entend et l'adaptabilité des formations aux attentes des intervenants des différents champs sanitaires et sociaux qu'il peut concerner lui donnent tout son intérêt. (Résumé de la revue) ENGLISH : The motivational interview (MI) is a method of communication centered on the patient, that has only been slightly developed in France up until now. It has now been clearly demonstrated to be useful in all therapeutic situations in which ambivalence and motivation are at the centre of the process of change. Addictions and therapeutic education are particularly concerned by this approach. The philosophy and principles of MI correspond to a series of well defined techniques to facilitate exploration and resolution of ambivalence in relation to change, which is considered to be normal in the motivational approach. After defining the principles and strategies of MI, this article describes some of the main techniques and tries to clearly define the role of MI in relation to other therapeutic approaches dealing with a change of behaviour. It also defines the modes of integration of MI into a more global therapeutic strategy. The strong points of MI are its flexibility in combination with other therapeutic modalities, the radical change in the patient-therapist relationship that it implies and the adaptability of formations to the expectations of professionals in various social and healthcare fields.
