Nous rapportons le cas d'une intoxication au cannabis chez une enfant de 16 mois, suite à l'ingestion accidentelle selon ses parents d'un morceau de résine de cannabis. À l'admission, l'enfant présente un coma (Glasgow 8/9), une pâleur, une tachycardie (168-192 bpm), une légère mydriase et une déshydratation modérée. Le dépistage urinaire est positif en cannabis. Aucune autre substance n'est détectée. L'enfant est hospitalisée pour surveillance. L'évolution est favorable et elle sort du coma en quelques heures. Elle quitte l'hôpital à j3. Des échantillons sanguins et urinaires ont été collectés 16 h après l'ingestion supposée. Un dosage du Delta9-tétrahydrocannabinol (THC) et de ses principaux métabolites, le 11-hydroxy-Delta9-tétrahydrocannabinol (11-THC-OH) et l'acide 11-nor-Delta9-tétrahydrocannabinol-9-carboxylique (THC-COOH) a été effectué par CG-SM. Une analyse capillaire segmentaire a également été effectuée, sur un échantillon de cheveux collecté à j3. Le THC, le THC-COOH, le cannabinol (CBN) et le cannabidiol (CBD) ont été quantifiés par CL-SM/SM dans 3 segments. Des concentrations très importantes en THC (98 µg/L), THC-OH (44 µg/L) et THC-COOH (920 µg/L) sont retrouvées dans le prélèvement sanguin. Des cannabinoïdes sont également présents dans tous les segments capillaires analysés, y compris du THC-COOH. L'intoxication aiguë sévère au cannabis chez cette enfant pourrait être associée à une exposition chronique. L'évolution étant rapidement favorable, des concentrations sanguines massives en cannabinoïdes ne semblent pas avoir de valeur pronostique péjorative dans l'intoxication cannabique en pédiatrie, mais un dosage sanguin et surtout une analyse segmentaire des cheveux se révèlent être importants en vue d'éventuelles procédures judiciaires. ENGLISH: We report a case of cannabis poisoning in a 16-month-old girl, after accidental swallowing of cannabis resin according to her parents. She presented coma (Glasgow Score 8/9), pale skin, tachycardia (168-192 bpm), slight bilateral mydriasis with unresponsive pupils, and mild dehydration. The suspected poisoning was confirmed by positive urine drug testing for cannabis. No other substance was detected. She was hospitalized for monitoring. No other complication was noted, and she came out of coma a few hours later. She left the hospital on day 3. Blood and urine samples were collected 16 hours after the ingestion. Quantification of Delta9-tetrahydrocannabinol (THC) and its major metabolites, 11-hydroxy-Delta9-tetrahydrocannabinol (11-THC-OH) and 11-nor-Delta9-tetrahydrocannabinol-9-carboxylic acid (THC-COOH) was performed by GC-MS in blood and urine. A segmented-hair analysis was performed too, on a hair sample collected on day 3, in order to explore her cannabis chronic exposure. THC, THC-COOH, cannabinol (CBN) and cannabidiol (CBD) were quantified by LC-MS/MS in three hair segments. Very high blood concentrations of THC (98 µg/L), THC-OH (44 µg/L), and THC-COOH (920 µg/L), were found. All hair segments were positive for cannabinoids, including THC-COOH. This severe cannabis poisoning could be associated with chronic exposure. The child recovered quickly, so massive cannabinoids concentrations seem to have no prognostic significance in pediatric poisoning by cannabis. However, determining blood levels of cannabis and segmented-hair analysis can be important for possible legal proceedings. |
