Objectifs : Décrire la prise en charge des patients traités par antiviraux pour hépatite C en Limousin en 2000 en se référant aux critères des conférences de consensus de février 1999 et février 2002. Mettre en place un plan dactions de formation et d'information des professionnels et des patients. Méthode : Enquête transversale rétrospective portant sur l'ensemble des patients ayant eu un traitement antiviral pour hépatite C en 2000. Nous avons repéré les patients par requêtes informatiques sur les bases de l'Assurance maladie, puis nous avons recueilli des informations directement dans les dossiers médicaux hospitaliers ou par lintermédiaire des prescripteurs : caractéristiques socio-démographiques, facteurs de risque de contamination, facteurs de pronostic évolutif et thérapeutique et devenir des patients après traitement. Résultats : En 2000, 129 patients ont été traités par antiviraux. Le recueil des données a été effectué pour 122 patients. Les facteurs de risque les plus fréquemment rencontrés étaient la toxicomanie (55 patients) et la transfusion sanguine (43 patients). Le bilan pré thérapeutique recherchant une co-infection par le VIH ou le virus de l'hépatite B, une consommation d'alcool excessive n'était pas systématiquement réalisé. Une bithérapie (interféron, ribavirine) concernait 90,2 % des patients. Le pourcentage de ces patients avec une charge virale indétectable, six mois au moins après l'arrêt du traitement, était de 47,6 %. Conclusion : Les résultats en terme de réponse virologique prolongée aux traitements étaient comparables voire supérieurs aux données de la littérature. Cependant, dans le dossier médical, un certain nombre d'informations essentielles au traitement, préconisées par la conférence de consensus, étaient absentes ou incomplètes. ENGLISH : Aims: To compare the care given to patients with hepatitis C who were treated with antiviral agents in the Limousin region in 2000 with respect to the February 1999 and February 2002 consensus conference guidelines. Put into place a training and information action plan for both healthcare professionals and patients. Method: We performed a cross-sectional, retrospective survey on all patients who were treated with antiviral agents for hepatitis C in 2000. The patients were located by using the healthfunds computer database. Clinical information (social and demographic characteristics, risk factors for contamination, therapeutic and prognostic factors and outcome following treatment) was gathered directly from their hospital medical records or from the physicians who prescribed their antiviral agents. Results: In 2000, 129 patients received antiviral treatment. We were able to study a total of 122 patients. The most frequently encountered risk factors were drug addiction (55 patients) and a blood transfusion (43 patients). Attending physicians did not systematically perform a pre-therapeutic workup for HIV, hepatitis B or alcohol abuse. 90.2 % of the patients received bi-therapy (interferon, ribavirine); 47.6 % of them had undetectable viral loads at least six months following the end of treatment. Conclusion: The long-term therapeutic responses with respect to viral load were comparable to or even superior to published reports. Nevertheless, a certain number of patients medical records did not contain part of all of the pertinent treatment information recommended by the consensus conference. |
