Cette étude examine les éventuels effets liés à l'adéquation intervenant-patient (les critères étant le sexe et l'ethnie), sur la perception de l'empathie chez l'intervenant, l'engagement du patient à son traitement, et la poursuite de l'abstinence pendant et après le traitement de la toxicomanie dispensé en consultation externe. Les résultats indiquent que l'adéquation intervenant-patient, sur le plan de l'ethnie et du sexe, est significativement associée à une augmentation de la perception de l'empathie chez l'intervenant dans toutes les catégories de groupes (sexe, ethnie et âge). Cependant, contrairement à toute attente, ce type d'adéquation n'est pas régulièrement associé à une augmentation de l'engagement du patient dans son traitement, ni à l'abstinence du patient, dans toutes les catégories de groupes (sexe, ethnie et âge). L'étude confirme la recommandation d'associer systématiquement des patients avec des intervenants du même sexe pour les femmes, les latino-américains et les patients de 35 ans et plus. Néanmoins, elle ne valide pas la recommandation visant à associer des patients à des intervenants congruents sur le plan ethnique (exception possible des patients du sexe féminin). L'étude montre aussi que, à l'exception des latino-américains, le choix d'associer un patient à un intervenant emphatique (quel que soit le sexe ou le groupe ethnique auquel celui-ci appartient) mènerait à des résultats de traitement plus favorables que l'association patient-intervenant basée uniquement sur l'adéquation sexe et ethnie. Cette étude souligne la nécessité de mener d'autres recherches examinant la relation patient-intervenant. Une meilleure compréhension de cette relation pourrait mener à une meilleure compréhension du processus de traitement de la toxicomanie et fournir des pistes pour améliorer globalement le traitement. ENGLISH : This study examined the possible effects of client-counselor gender and ethnic congruency on perceived counselor empathy, client engagement in treatment and the maintenance of abstinence during and after out-patient drug treatment. The findings indicate that client-counselor gender and ethnic congruence were significantly associated with higher levels of perceived counselor empathy for all gender, ethnic, and age groups. Contrary to expectation, however, client-counselor gender and ethnic congruence were not consistently associated with higher levels of treatment engagement and abstinence for all gender, ethnic, and age groups. The findings support a recommendation of routinely matching clients to counselors of the same gender for women, Latinos, and clients 35 years or older, but they do not support a recommendation of matching clients to ethnically-congruent counselors, with the possible exception of female clients. The findings also indicate that, with the exception of Latino clients, a policy of matching a client to an empathic counselor regardless of gender or ethnicity of the counselor would lead to a more favorable treatment outcome than would a policy of matching clients solely to gender and ethnically-congruent counselors. This study highlights the need for additional studies examining the client-counselor relationship. Further understanding of this relationship could lead to a greater understanding of the drug treatment process and suggest ways to generally improve the delivery of drug treatment.
