Le présent rapport restitue les résultats de l'évaluation de processus et d'efficacité du programme « Avenir sans Tabac » (AsT) porté par le Souffle 64 (Pau). Ce programme compte parmi les 5 projets prometteurs sélectionnés par la Cipca. Son objectif est d'aider à l'arrêt ou à la diminution de la consommation des jeunes fumeurs de tabac et de cannabis. Sa stratégie d'intervention repose sur la technique de l'entretien motivationnel (EM) dans le cadre de la participation 1) à un stand Tabac-Cannabis (EM bref) et 2) à un atelier collectif d'aide à la motivation (EM de groupe) proposés dans chaque établissement partenaire des Pyrénées-Atlantiques (Lycées, CFA). Nous avons recueilli des indicateurs d'efficacité auprès de fumeurs récents de cigarettes et/ou de cannabis selon qu'ils ont été exposés ou non à l'intervention AsT (stand seul ou stand+Atelier). Les données ont été collectées avant l'intervention et 3 mois après l'intervention. Nous avons constaté que la participation à un stand Tabac-Cannabis était associée à une diminution du niveau de consommation récente des jeunes fumeurs de cigarettes et de cannabis. L'intervention avait aussi des retombées positives sur les déterminants de consommation chez les fumeurs de cigarettes (intention d'arrêt, auto-efficacité, consommation perçue des pairs). L'impact positif sur les déterminants mesurés 3 mois après l'intervention était d'autant plus élevé que la qualité de l'EM l'était. Pour les participants de l'atelier (recrutés lors du passage au stand), AsT a eu un effet positif sur les déterminants de la consommation (surtout chez les fumeurs de cigarettes) mais n'a pas eu d'effet sur le niveau de consommation. Globalement, nous n'avons observé aucun effet du programme sur les tentatives d'arrêt et l'abstinence. Il semble donc que le stand Tabac-Cannabis soit une intervention efficace pour réduire les consommations. L'atelier collectif, quant à lui, est un dispositif potentiellement prometteur mais pas encore stabilisé quant à sa mise en oeuvre ; il devra donc faire l'objet d'ajustements. ENGLISH: This report presents the results of the process and effectiveness evaluation of the programme "Avenir sans Tabac" (AsT - Tobacco Free Future) supported by the association Souffle 64 (Pau), which is one of five promising projects selected by the Interministerial Commission for the Prevention of Addictive Behavior (CIPCA). Its purpose is to help stop or reduce the consumption of tobacco and cannabis among young smokers. Its intervention strategy is based on the Motivational Interviewing (MI) technique in the context of participation in 1) a Tobacco-Cannabis stand (brief MI) and 2) a collective motivation workshop (group-based MI) offered in each of the Pyrénées-Atlantiques partnerinstitutions (high schools, Apprentice Training Centres). We have collected effectiveness data from recent smokers of cigarettes and/or cannabis depending on whether or not they were exposed to the AsT intervention (stand only or stand + workshop). Data were collected before the intervention and 3 months after the intervention. We found that participation in a Tobacco-Cannabis stand was associated with a decrease in the recent consumption level of young cigarette and cannabis smokers. The intervention also had a positive impact on the determinants of consumption among cigarette smokers (intention to stop, self-efficacy, perceived peer consumption). The positive impact on the determinants measured 3 months after the intervention was as high as the quality of the MI was. For workshop participants (recruited at the stand), AsT had a positive effect on the determinants of consumption (especially for cigarette smokers) but had no effect on the level of consumption. Overall, we did not observe any effects of the program on quit attempts and abstinence. It seems that the tobacco-cannabis stand is an effective intervention to reduce consumption. The collective workshop, meanwhile, is a potentially promising system but not yet stabilized as to its implementation; it will therefore have to be adjusted. |
