Contexte : Depuis 2011, on observe un essor de la consommation du sulfate de morphine (SdM) à visée substitutive de l'héroïne, malgré l'existence de la méthadone et de la buprénorphine haut dosage (BHD) dans le traitement de substitution des opiacés (TSO). Objectif et Méthode : L'objectif de l'étude était d'explorer les raisons de cet essor en tentant d'identifier la place du SdM en alternative aux médicaments de substitution des opiacés (MSO) existants à travers l'expérience d'usager.e.s de SdM en France. La recherche s'appuyait sur une méthode qualitative par entretiens semi-dirigés. Résultats : 11 personnes, s'auto-définissant comme consommateur.rice.s de SdM à visée substitutive, ont été rencontrées entre mars et novembre 2016. La plupart des participant.e.s l'utilisait par voie intra-veineuse. Le SdM pouvait être identifié comme un poison face aux conséquences néfastes liées aux injections et à la recherche du produit. Le SdM était un remède lorsqu'il était utilisé à visée auto-thérapeutique. Sa spécificité résidait dans un effet ressenti significatif à la prise, et une diminution importante du craving en dépit d'une tolérance élevée. Les attentes se concentraient sur une meilleure reconnaissance du SdM comme MSO, une prise en compte de la problématique de l'injection et un accueil bienveillant non stigmatisant par le corps soignant. Discussion : La fonction de la consommation variait, entre poison et remède, au cours du parcours de l'usager.e de SdM, de la même façon que ce qui est décrit dans la littérature pour les autres consommateurs.rices de MSO. Ce polymorphisme des consommations, confortait l'importance d'une prise en charge pluridisciplinaire de la toxicomanie. L'effet ressenti interrogeait la fonction hédonique attribuée à l'usage du SdM et semblait sous-tendre la recherche d'une homéostasie, pouvant expliquer en partie le mésusage d'un tel médicament. Le SdM investit en objet de transition dans le soutien au sevrage à l'injection, ouvrait la question de l'émergence d'une substitution injectable et la nécessité de se saisir de l'injection comme une problématique autonome. Conclusion : Le discours des usager.e.s conforte la nécessité d'ouvrir le panel thérapeutique disponible, à travers l'émergence de nouvelles formes galéniques de MSO, et notamment injectables. Ce travail peut constituer l'approche exploratoire nécessaire à l'élaboration de recherches plus amples en direction des usager.e.s de SdM pour établir son véritable intérêt dans le cadre des TSO. ENGLISH: Context: Since 2011, we observe a major rise of slow-released oral morphine (SROM) use as a substitute to heroin, even though methadone and buprenorphine at high dosage already exist as opioid maintenance treatment. Objective and Method: The objective of this study was to explore the reasons of this rise by trying to identify SROM as an alternative to the available opioid maintenance treatment through the experience of SROM users in France. The research was based on a qualitative method using semi-directed interviews. Results: 11 people defining themselves as consumers of SROM as an opioid maintenance treatment were questioned between March and November 2016. Most of the participants self-injected it. The SROM could be identified as a poison due to the harmful consequences linked to the injection itself and to the seeking of the product. The SROM was considered a remedy when it was used as self-treatment. Its distinctiveness resided in the significant effect felt when taken, and in a major fall of the craving despite an elevated tolerance. Their expectations were focused on a better recognition of the SROM as an opioid maintenance treatment, a taken into account of the injection issue and a kind welcome from the caregiving profession, this being without stigmatisation. Discussion: The consumption function varied between poison and remedy all along the users paths, the same way that it is described within the literature for users of opioid maintenance treatment. This consumption polymorphism reinforced the necessity of a pluridisciplinary management of drug addiction. The felt effect questioned the hedonic function attributed to the use of SROM and seemed to underlie the seeking of homeostasis, explaining partly the misuse of that kind of treatment. The SROM invested as a transition object in the support of injection withdrawal, opened up the question of the emergence of injected maintenance treatment and the necessity of taken into account the injection as its own issue. Conclusion: The users discourse reinforced the necessity of opening the range of treatments available, through the emergence of new galenic forms of SROM, especially injectable ones. This work can constitute as the exploratory approach necessary to elaborate more extensive research directed towards SROM users, in order to identify its genuine interest as a maintenance drug treatment. |
