À Marseille, Toulouse, Lille et Paris, démarre une étude de la plus haute importance : démontrer que l'accès à un logement stable pour des personnes exclues s'avère un des meilleurs moyens de les traiter. De quoi s'agit-il ? Au Canada et aux États-Unis, le "housing first" ("Un chez soi d'abord") a gagné dans toutes les études (qui sont nombreuses) contre le "Treatment first". Dans le housing first, les personnes à la rue souffrant de troubles mentaux sévères et éventuellement d'addictions se voient proposer un accès au logement immédiat à condition d'accepter un suivi au domicile de l'équipe pluridisciplinaire chargée de ce traitement communautaire actif. Le Treatment first, lui, reporte l'attribution d'un logement permanent aux malades sans abri ayant participé avec succès à un programme d'observance et d'abstinence... On ne peut que se féliciter, d'une part, de procéder à une telle étude, sérieuse et scientifique, sous nos climats : nous pêchons trop souvent en France par l'absence d'évaluation ; mais aussi, d'autre part, il serait temps qu'il soit communément admis que l'hébergement est thérapeutique. En effet, toute démarche de réhabilitation psychosociale commence obligatoirement par l'hébergement qui s'avère avant tout le lieu où le sujet va pouvoir se reconstituer et échafauder ses projets de vie. C'est ce que, depuis plus de 30 ans, nous tentons de faire admettre en défendant la notion "d'hébergement thérapeutique" ! Les appartements thérapeutiques (ou protégés, ou associatifs, etc.) réalisent la formule la plus efficace pour les patients les plus lourds. Rappelons que ce qui est thérapeutique dans les appartements thérapeutiques c'est bien entendu le "contrat thérapeutique" passé entre l'usager et l'équipe soignante ou médicosociale. Ce contrat engage forcément les uns et les autres dans un projet de vie et de soins et la continuité du suivi est ainsi assurée, y compris au niveau du domicile. ENGLISH: In Marseille, Toulouse, Lille and Paris, a study of the highest importance has begun to demonstrate that access to basic housing for people who have been excluded is one of the best ways to deal with their problem. What does this involve? In Canada and the United States, the "Housing First" won in all the studies (which were numerous) as opposed to "Treatment First." In Housing First, people who are in the street suffering from severe mental disorders and possibly addiction are offered immediate access to housing provided they accept a follow-up at home by the multidisciplinary team who are responsible for active community treatment. The Treatment First approach involves the granting of permanent housing for homeless patients who have successfully participated in a program of compliance and abstinence. We can only congratulate this initiative to conduct such a serious and scientific study in the current climate. In France, we too often sin due to the lack of assessment, and it is time that we commonly accept that accommodation is therapeutic. In fact, any approach to psychosocial rehabilitation must begin with accommodation, which is primarily the place where the individual will be able to reconstruct and attain their life ambitions. This is what we have tried to admit for over 30 years in order to defend the notion of "Therapeutic Accommodation"! Therapeutic apartments (or protected or associated, etc.) which is the most effective formula for the most needy patients. Remember that what is therapeutic in therapeutic apartments is of course the "therapeutic contract" signed between the user and a healthcare or medico-social team. This contract obviously commits each party to a project for life as well as assuring the continuity of patient care and follow-up including home care. |
