Le PAACT - Processus d'accompagnement et d'alliance pour le changement thérapeutique - est un manuel qui propose une formalisation "pratique" du travail d'alliance avec un jeune, souvent venu sur injonction d'un adulte, et de l'accompagnement aux changements de son comportement d'usage. Il décrit une formalisation des pratiques pour optimiser l'intervention thérapeutique dès les premiers stades d'un usage problématique (abus et dépendance). L'élaboration de ce manuel a respecté les différentes étapes du processus complexe de construction d'un modèle thérapeutique : après une formalisation empirique de la thérapie, celle-ci a fait l'objet d'une étude comparative au sein d'un essai clinique randomisé, approfondie avec un travail doctoral. Les résultats obtenus sur la réduction de l'usage et l'observance ont montré l'intérêt de la formalisation de la thérapie et conduit à produire et diffuser un manuel : le PAACT. Pour illustrer cette thérapie, la situation de Paul, 16 ans, usager de cannabis, est présentée à différents moments de la prise en charge. ENGLISH: The PAACT (coaching and therapeutic process to build alliance and changes) is a manual that formalizes psychotherapeutic practices. The goal is to help the therapist to build the alliance with a youngster, who often comes to therapy on request of an adult, and to coach an adolescent to make him change his addictive behavior. This formalization of practice improves therapeutic interventions in the early stages of substance abuse and dependence. The construction of this manual has respected the different stages of the complex process of building a therapeutic model: it starts with an empirical description of usual practices which were the subject of a comparative study in a randomized clinical trial (The INCANT Study) associated with a PhD work. The good results of a formalized therapy on cannabis abuse and on patient adherence highlighted the interest of therapeutic processes formalization, and led to produce and distribute the PAACT handbook. To illustrate this therapy, the situation of Paul, a 16 year old cannabis user, is presented at different stages of the treatment. |
