Contexte : l'association buprénorphine haut dosage (BHD) et benzodiazépines (BZD), bien que dangereuse, est souvent retrouvée (18 à 50 %) chez les consommateurs de BHD. Objectif : déterminer les facteurs explicatifs de cette coprescription. Méthode : étude qualitative par entretiens semi-directifs individuels auprès d'un échantillonnage représentatif de médecins prescripteurs de BHD. Résultats : dix médecins généralistes exerçant en milieu urbain, rural ou semi-rural ont été interviewés. Ils suivaient entre cinq et 60 patients substitués par BHD. Tous ont déclaré connaître le risque délétère de l'association BHD-BZD. Ils n'initiaient quasiment jamais de BZD chez un patient substitué par BHD, mais reconduisaient souvent le traitement par BZD préalablement instauré. Notre étude a permis d'identifier trois profils de médecins prescripteurs de BHD : empathique, intermédiaire ou autoritaire. Ce profil influence certainement le renouvellement ou non de cette coprescription. Le toxicomane consomme souvent des BZD en raison d'une anxiété fréquemment retrouvée chez ces patients. Conclusion : les médecins prescripteurs de BHD connaissent la dangerosité de l'interaction entre BHD et BZD et en tiennent compte le plus souvent. La formation doit intégrer l'approche relationnelle et le sevrage ambulatoire des BZD. Le développement de liens entre médecins généralistes, psychiatres et psychologues est indispensable. Le problème réside également dans la banalisation de la prescription des BZD par l'ensemble des médecins français. ENGLISH: Background: although dangerous, the association of high dosage buprenorphine (BHD) and benzodiazepine (BZD) is often reported (from 18 to 50%) among the consumers of BHD. Objective: to determine the reasons explaining this co-prescription by general practitioners (GPs). Method: a qualitative study has been conducted. Semi-directive interviews with a representative sampling of GPs, who often prescribe BHD, were carried out. Results: ten GPs from urban, rural and semi-rural areas were interviewed. Each of them was following the progress of five to 60 patients treated with BHD as maintenance therapy. All of them declared they knew the deleterious risk of the association BHD-BZD. They never started the prescription of BZD for a patient treated with BHD. However they often renewed the BZD treatment if already prescribed. Our study underlined three profiles of BHD prescribers: empathic, intermediate and authoritarian. This profile most likely affected the renewal of this co-prescription. The drug addict often consumed BZD because of anxiety. Conclusion: BHD prescribers are aware of the dangerousness of the interaction between BHD and BZD and take it into account when treating their patients. Education must insert relational approach and ambulant weaning of BZD. The development of links between general practitioners, psychiatrists and psychologists is necessary. The main issue is the trivialization of BZD prescription by all French doctors. |
