Le soutien psychothérapique est un facteur essentiel de l'aide au sevrage tabagique. Le tabagisme devrait désormais être vu comme un comportement automatique acquis, consistant en l'autoadministration d'une substance d'abus sous l'influence d'un besoin de nature pharmacologique et de renforcements comportementaux secondaires d'origine interne ou environnementale. Tout se passant ainsi dans un registre indépendant de la volonté, l'appel à des efforts de volonté ne devrait plus constituer la base de la psychothérapie, l'arrêt du tabac apparaissant plutôt comme le fruit d'une longue maturation inconsciente aboutissant à une prise de décision. Cette maturation pourrait être aidée en attirant l'attention du patient sur certaines motivations privilégiées : désir de libération d'un sentiment d'esclavage, exemple pour l'entourage, désir d'amélioration de l'état de santé actuel plutôt qu'en brandissant la menace de risques futurs pour la santé. Une attitude éducative, informative sur les mécanismes de la dépendance, mettant l'accent sur leur automaticité peut déculpabiliser les échecs. L'arrêt brutal est à conseiller, et les conseils habituellement prodigués sont passés au crible de la critique. La stabilisation de l'arrêt est présentée comme un apprentissage actif, consistant à apprendre à vivre sans tabac toutes les situations antérieurement renforçatrices, ce qui donne à l'arrêt du tabac l'image positive d'un apprentissage et non celle d'une frustration passivement subie. ENGLISH: Psychotherapy is very important in helping smokers to quit. Nowadays, smoking should be regarded as an acquired automatic behavior consisting in self-administration of an addictive substance as a result of a pharmacologie need and of secondary behavioral reinforcements of internal or environmental origin. Because this entire process takes place outside of the realm of volition, calling on the subject to exercise will power should no longer form the basis of psychotherapy. Smoking cessation is, on the contrary, the fruit of a long process of unconscious maturation that leads to the decision to quit smoking. Support to this process can be provided by focusing attention on especially strong motivations, including the desires to be free from a form of bondage, to be a satisfactory role model for ethers, and to improve current health, rather than by threatening or frightening the subject with references to hazards to future health. By educating and informing the smoker on the mechanisms of dependency and putting emphasis on the automatic nature of the dependency process, feelings of guilt experienced by would-be quitters who have failed can be assuaged. Immediate complete withdrawal is preferable. The usual recommendations are critically reviewed. Maintenance of abstinence is viewed as an active learning process during which the subject must learn to live through all the previously reinforcing situations without using tobacco. This depiets quitting positively, as a learning achievement, rather than negatively, as a passively borne frustration. |
