Le groupe de travail sur l'hébergement thérapeutique de la commission Addictions a auditionné l'Association nationale des intervenants en toxicomanies et addictions (ANITeA) sur sa conception du traitement résidentiel. Ce travail, après une mise en perspective historique évoquant le souvenir des 'postcures' de naguère, présente un état des lieux de ce dispositif dans notre pays, en s'appuyant sur l'expérience professionnelle collective et les données de la littérature : types de structure et de projet thérapeutique (centre thérapeutique résidentiel, communauté thérapeutique, service de soins de suite et de réadaptation), indications et critères de placement, articulation avec l'ambulatoire et l'hospitalier, durée de prise en charge, évaluation, besoins actuels aussi bien quantitatifs que qualitatifs. En définitive, il apparaît que si le traitement résidentiel ne doit plus être le standard de traitement des addictions comme cela a pu être le cas par le passé, il répond à de réels besoins insuffisamment satisfaits, notamment ceux de patients présentant d'importants problèmes aux plans psychologique et social et dont la situation ne peut être prise en charge en ambulatoire dans de bonnes conditions de sûreté ou d'efficacité. (French) ENGLISH: The working party on therapeutic accommodation of the Addiction commission heard the Association national des intervenants en toxicomanies et addictions (ANITEA) presentation on its view on residential treatment. This study, after presenting the historical background recalling the age of "post-withdrawal" centres, presents a review of this system in France based on a collective professional experience and data from the literature: types of structures and therapeutic project (residential treatment centre, therapeutic community, convalescence and rehabilitation units), indications and criteria of placement, coordination with outpatient and hospital medicine, duration of management, evaluation, and current quantitative and qualitative needs. Although residential treatment should no longer be the standard treatment for addictions as it was in the past, it can meet real insufficiently satisfied needs, especially those of patients with major psychological and social problems, whose situation cannot be managed on an outpatient basis under good conditions of safety or efficacy. |
