FRANÇAIS : Objectif: L'étude prospective, réalisée par le service médical de l'assurance maladie du régime général de Pau de 1996 à 1999, visait a établir le bilan du suivi actif d'une population de patients traités en ville pour leur dépendance à des substances ou médicaments psycho-actifs. Méthodes : Les patients ont été inclus a la suite d'un premier état des lieux, réalisé en 1996 sur la pharmacodépendance aux substances psycho-actives en Béarn et Soule, ou ont été intégrés au fur et à mesure de signalements ultérieurs, permettant néanmoins une observation minimale d'une année. Ces malades, tous dépendants aux opiacés, présentaient initialement une errance auprès de plusieurs médecins et plusieurs pharmaciens et se procuraient des médicaments pouvant faire l'objet d'abus de consommation ou d'usage détourné. Les interventions du service médical ont consisté à fidéliser le patient auprès d'un médecin et d'un pharmacien et à réaliser un contrat de soins en concertation avec les professionnels de santé et le malade lui-même. Résultats : En juillet 1999, ce suivi au long cours avait concerné 42 patients et aboutissait au constat suivant : prise en charge médicale et pharmaceutique régulée s'accompagnant d'une amélioration de l'état sanitaire et social pour 34 d'entre eux ; persistance d'un état critique pour 6 autres ; arrêt du suivi pour 2 patients perdus de vue ; respect du contrat de soins par 8 malades sur 10. Conclusion : Ce travail souligne le rôle de régulation que l'assurance maladie, par son service médical, peut jouer dans une filière de soins réputée complexe à mettre en oeuvre, en raison notamment de la nature même de l'affection à prendre en charge et du comportement des patients qui en sont les victimes. (Résumé de la revue) ENGLISH : Aims: The aim of this prospective study by the medical service of the general scheme of health-care coverage (régime général de 1'assurance maladie) of Pau from 1996 to 1999 was to determine the results of the active follow-up of an out-patient population treated for psychoactive drug abuse. Methods: The study population was comprised by a group patients who had been the object of an inventory of patients addicted to psychoactive substances in Béarn and Soule in 1996 and a second group of patients who were added to this list as soon as their existence became known and who were able to be followed for a minimum of one year. Prior to the study, these patients consulted a number of different physicians and chemists in order to procure sufficient quantities of potentially addicting drugs or other substances for illicit use. The role of the medical service consisted in inciting patients to receive care from just one health professional and in establishing a contract between each patient and the concerned physician or chemist. Results: As of July 1999, we were able to study the long-term follow-up of 42 patients and found the following results: medical and pharmaceutical care resulting from the contracts produced improved health and social well-being in 34 patients, 6 other patients failed to significantly modify their behavior, 2 patients were lost to follow-up, 8 out of 10 patients who were studied fulfilled the contract they signed. Conclusions: This study underscores the regulatory role that healthcare coverage (assurance maladie) can play, through its medical service, in a health network which has the reputation of being extremely difficult to implement because of the very nature of the disorder involved and the behavior of its victims, addicted patients.
