A study was conducted in 31 Karen tribal villages in northern Thailand in 1999-2000 to address the question of why some villages have a high prevalence of illicit drug use and associated problems while others do not. Data were gathered from village leaders, residents, and field workers. Interviews were also held with addicts in a treatment center. Analyses focused on differences between "High Drug" (n = 16) and "Low Drug" (n = 15) villages, which were defined by a cluster analysis of opium and methamphetamine use by various age groups, presence of drug dealers in the village, and an overall assessment of addiction in the village. Overall, few village-level variables were significantly related to drug use in the villages, although there was some support for the hypothesis that larger and more economically developed villages manifested higher levels of drug involvement. Some of the specific findings which were significant included greater access to elementary education for High Drug villages, compared with Low Drug villages. Alcoholism was significantly related to village status also, with 63 percent of High Drug villages and only 23 percent of Low Drug villages reporting having more than four alcoholic residents.
