Objectif. - En France, la prise en charge des personnes dépendantes aux opiacés a été profondément modifiée par la mise à disposition des médecins généralistes en 1996 d'un traitement de substitution par voie sublinguale, la buprénorphine haut dosage (Subutex°). Afin d'évaluer les bénéfices de ce traitement, une étude épidémiologique prospective, multicentrique a été conduite en France, à partir de mai 1996, chez 105 médecins généralistes, avec un suivi sur 2 ans. Méthodologie. - Elle a concerné une population ambulatoire de patients dépendants aux opiacés qui débutaient ou avaient débuté récemment un traitement de substitution par buprénorphine haut dosage. Ces patients pris en charge par des médecins généralistes ont été suivis avec un recueil à 1, 3, 6, 12, 18 et 24 mois des données usuelles d'évaluation des patients dépendants aux opiacés. Le critère d'évaluation principal était le suivi par le même médecin généraliste et le maintien du patient dans le système de prise en charge à 2 ans. Chez les patients ayant rempli ce critère, plusieurs critères secondaires ont été évalués : modalités de prescription du traitement de substitution, situation sociale, séroconversions (VIH, VHB, VHC). Conclusion. - Après 2 ans de suivi d'une cohorte de 909 patients dépendants aux opiacés, le maintien dans le système de soins a été observé près de 7 fois sur 10 ; principalement sous la responsabilité du médecin généraliste initial ou, plus rarement, par un autre médecin. Parmi les 508 patients toujours suivis par le même médecin, le maintien du traitement par la buprénorphine haut dosage a été observé plus de 8 fois sur 10. Chez ces mêmes patients, il a été noté une amélioration significative du statut social (logement, activité professionnelle), une réduction significative de la consommation de produits illicites et une amélioration significative de l'adaptation sociale (score GAF-S) et de la gravité de la toxicomanie (Score TMSP). (A partir du résumé d'auteur) ENGLISH : Aim of the study. - Care for opioid users changed greatly in France in 1996 when general practitioners (GP) were allowed to prescribe high-dose sublingual buprenorphine (Subutex°) for maintenance treatment of major opioid dependence. In order to evaluate treatment benefits, a prospective epidemiological 2-year follow-up was initiated in May 1996 with the participation of 105 French GPs. Methods. - A cohort of outpatient opioid users who started high-dose sublingual buprenorphine maintenance therapy at study onset or who had recently started were included in a prospective epidemiological study by GPs involved in management of drug abusers. Patients were followed for 2 years with collection of standardized information at 1, 3, 6, 12, and 24 months. The main evaluation criteria were follow-up by the same GP throughout the study and retention in the care system 2 years later. For patients who fulfilled these criteria, secondary end points were analyzed: information about buprenophine prescription, social status, and hepatitis B and C and HIV seroconversions. Conclusion. - This two-year follow-up of 909 opioid users showed that nearly 70% of the patient remained within the healthcare system, mainly with the same GP or more rarely with another practitioner. Among the 508 patients still followed by the same GP, maintenance treatment with high-dose buprenorphine was observed in more than 80% of the patients. These patients bad a significantly improved social status, a signnificant decrease in drug intake and a significant improvement in their social adap- tation and severity of drug abuse.
