Objectif : Evaluer et décrire la population de bénéficiaires du régime général d'assurance maladie ayant été remboursés de médicaments psychotropes en France métropolitaine, en 2000. Méthode : Etude transversale sur les bases de données de remboursement de l'assurance maladie à partir du codage de la pharmacie. La population étudiée a été constituée par sondage aléatoire à partir du numéro de sécurité sociale des personnes. Les patients sélectionnés ont eu au moins un psychotrope remboursé au cours de l'année 2000. Pour chaque classe thérapeutique, les variables retenues identifiaient le nombre de bénéficiaires d'exonération du ticket modérateur pour affection de longue durée, de couverture maladie universelle, de pension d'invalidité, la nature du suivi médical et le nombre de remboursements au cours de l'année. Résultats : En 2000, près d'un quart de la population protégée par le régime général stricto sensu (24,5 %) a bénéficié du remboursement d'un médicament psychotrope. Les taux spécifiques variaient avec l'âge et le sexe (33 % des hommes et 55 % des femmes après 70 ans). Les anxiolytiques étaient les plus utilisés (17,4 %), suivis des antidépresseurs (9,7 %), des hypnotiques (8,8 %), des neuroleptiques (2,7 %), des médicaments utilisés dans la dépendance alcoolique (0,5 %) et du lithium (0,1 %). Le taux annuel de consommateurs de psychotropes, ayant eu au moins quatre remboursements pour une même classe thérapeutique, était de 11,2 % (anxiolytiques 7,0 %, antidépresseurs 4,9 %, hypnotiques 3,7 %). Conclusion : Malgré l'absence d'information sur le contexte clinique de ces prescriptions, ces données permettent cependant d'identifier un seuil minimum de consommateurs et de proposer des indicateurs de suivi de cette consommation aisément reproductibles. ENGLISH: Aims: To describe and assess the population of beneficiaries affiliated with the general health fund who received reimbursement for psychotropic medications in metropolitan France in 2000. Method: We performed a cross-sectional study using the health funds reimbursement data based on the pharmacy coding system. Patients were enrolled by random selection of the social security numbers of patients who received at least one reimbursement for a psychotropic medication during the year 2000. In each therapeutic class, we noted: the number of affiliates exempted from co-payments due to a long-term disorder, those covered by universal medical coverage (CMU), patients on medical disability, the type of medical follow-up they received and the total number of reimbursements they had requested during the study period. Results: During the year 2000, almost one-fourth of the population covered by the general health fund stricto sensu (24.5%) was reimbursed for a psychotropic medication. Specific reimbursement rates varied according to age and sex (33% of all males and 55% of all females older than 70 years). Anxiolytic agents were the most frequently prescribed psychotropic medications (17.4%) followed by antidepressors (9.7%), hypnotics (8.8 %), neuroleptics (2.7%), drugs used to combat alcohol addiction (0.5%) and lastly, lithium (0.1%). 11.2% of all affiliates received at least four reimbursements for the same therapeutic class (anxiolytics: 7.0%, anti-depressors: 4.9%, hypnotics: 3.7%). Conclusion: Although this study method provides no clinical information on prescriptions, the data we obtained allowed us to identify easily reproducible information on the minimum threshold of patients treated with psychotropic agents and to suggest future follow-up indicators. |
