En France, 200 généralistes ont été tirés au sort parmi les 52000 recensés par la CNAM. Ils ont rempli entièrement un long questionnaire de santé publique dans lequel l'alcoologie était prépondérante. Le taux de réponses est de 83 %. Les généralistes répondants ont déclaré majoritairement un manque de formation, de savoir-faire, de soutien, une démotivation, malgré des connaissances plutôt satisfaisantes en alcoologie et la conscience de l'importance du problème en médecine générale. Trois quarts d'entre eux étaient peu à l'aise pour poser des questions aux consommateurs à problèmes ; 80 % estimaient que leur conseil ne serait pas suivi d'effet et que leur patients pourraient s'indigner de leurs questions ; 83 % regrettaient l'absence de rémunération pour le temps consacré à la prévention. Une majorité (60 %) se jugeaient incompétents pour intervenir auprès des patients alcoolodépendants. Quant aux perspectives, une grande majorité des généralistes considéraient qu'ils pourraient être personnellement aidés si les patients étaient demandeurs de conseils et prêts à payer pour être conseillés, s'il était prouvé qu'une intervention brève était efficace, s'il existait des programmes de formation à l'intervention brève pour les consommateurs à problèmes (non dépendants). Avec un soutien, 80 % d'entre eux se sentaient capables de traiter les patients alcoolodépendants. Les généralistes français sont traditionnellement des soignants, et parler d'alcool reste encore un sujet tabou, peu enseignés mal repéré et évacué. L'intervention brève est encore mal connue. De continuels efforts et incitations des pouvoirs publics, de l'université, des instances de formation médicale post universitaire, des médias, ont, depuis des décennies, amélioré l'approche de ces patients avec qui il faut tout simplement trouver les mots pour le dire... C'est à un champ clinique et conceptuel passionnant que nous invitent nos patients addictés. (Résumé d'auteur.) ENGLISH : In France, 200 GP who had drawn lots among 52000 participated. They replied to a list of questions about public health. A lot of them were about alcohol. Eighty three percent GP returned the questionnaire. The most of them indicated that they didn't have enough education, ability, support and motivation for taking care of patients who drink alcohol despite good knowledge in alcohology. 75% of the GP who replied judged they were not used ask questions about alcohol drinking; 80% estimated that their counsels would have no effect and that their patients would be indignant if they talked about alcohol; 83% regretted the lack of remuneration for doing prevention; 60% indicated they were not competent to do something for alcohol-dependent patients. The most of the GP who replied thought they could be helped if patients ask themselves counsels and want to pay for that; if evidence exists that brief interventions is efficacy; and if brief intervention's education exists. With more support, 80% of the GP considered themselves able to treat patients alcohol-dependent patients. Talking alcohol is still taboo, not enough taught and badly detected. Brief intervention is still not enough known. Lots of things have been done for more ten years to improve the approach of alcohol's patients to which we have just to find the words to say it.
