Introduction - Le ministère de la santé a évalué l'application de la règlementation à l'égard du tabac dans les hôpitaux publics français au printemps 2006. Méthode - Une enquête sur échantillon aléatoire stratifié a été menée en 2006. Les taux de tirage ont été raisonnés pour tenir compte des tailles d'établissements. Les personnels, les patients et les directeurs de 316 établissements ont répondu à un questionnaire auto-administré, établissant le taux de réponse à 70 %. Résultats - En 2006, 67 % des directeurs et 47 % des personnels considéraient que le tabac était « un problème » dans leur établissement. La taille des hôpitaux, la taille des communes d'implantation, la spécialisation en psychiatrie augmentent généralement les difficultés. À la date de l'enquête, l'interdiction totale du tabac dans les hôpitaux était une perspective approuvée par 73 % des directeurs et 46 % des personnels. Discussion - La situation française ne faisait pas exception en Europe en 2006. Conclusion - Environ deux-tiers des établissements révèlent des difficultés d'application de la règlementation de 1992. L'interdiction totale du tabac dans les bâtiments hospitaliers décrétée à compter du 1er février 2007 mettra probablement en évidence le rôle propre négatif de l'absence de dispositions particulières aux établissements de santé dans le décret de 1992. ENGLISH : Introduction - In French public hospitals, the implementation of the tobacco smoking limiting rules was evaluated in Spring 2006 by the Ministry of Health. Methods - A survey was carried out on a stratified random sample. Randomization ratio was reasoned to take into account the hospital size. Staff, patients and Chief hospital directors from 316 hospitals filled out a self-administered questionnaire, achieving a final average response rate of 70% among the target sample. Results - In 2006, 67% of directors and 47% of the staff considered tobacco smoking in their hospital as « a problem ». Hospital size, urban size context, psychiatric activity generally increase the troubles. At the surveys time, the project for a total ban on smoking in hospitals was approved by 73% of hospitals directors and 46% of the staff. Discussion - In 2006, the French situation was not distinctive among other European countries. Conclusions - Around two-thirds of the hospitals reveal the existence of troubles to implement the 1992 regulation. The complete smoking ban in hospitals premises was enacted from 1 February 2007, and this is likely to bring to light the negative effect of having withdrawn the former specific regulations on health facilities.
