L'étude a été menée en maison d’arrêt. Les patients décrits dans l’enquête sont donc des prévenus en attente de jugement et des condamnés dont le reliquat de peine est inférieur à un an ou en attente d’affectation dans un établissement pour peine. Elle a permis de décrire le profil général des détenus dépendants aux opiacés mais aussi de mener une étude prospective, la première du genre en France, sur une période de 24 mois, de façon à évaluer le taux de réincarcération des détenus ayant bénéficié de TSO, par rapport au groupe-témoin des détenus qui n’en ont pas bénéficié pas au moment de leur entrée en détention. Cette analyse a permis de pointer les facteurs associés à la réincarcération. Enfin, l’enquête s’est attachée à mettre en exergue les différences de profils entre les personnes bénéficiant d’un TSO à l’entrée en détention (pour la première fois ou non) et celles dont la substitution a été interrompue ou non renouvelée à l’entrée en prison, qui sont autant d’éléments utiles, en termes d’aide à la décision, pour repérer les signes prédictifs d’un abandon du traitement (ou anticiper un abandon du traitement). (Résumé d'éditeur) ENGLISH: The study was carried out in remand centres. The patients described in the survey are consequently prisoners awaiting trial and convicts whose remaining sentence is less than one year or who are awaiting a transfer to an établissement pour peine (penal institutions, detention centres, etc. for convicted prisoners). It has made it possible to describe the general profile of opioid-dependent prisoners but also to carry out a prospective study, the first of its kind in France, over a period of 24 months in order to assess the level of reincarceration among prisoners having received OST compared to a control group of prisoners who did not receive an OST when beginning their period of detention. This analysis made it possible to identify the various factors associated with reincarceration. Finally, the survey has sought to highlight the differences in profile between those people receiving an OST when their period in detention began (for the first time or otherwise) and those whose substitution was interrupted or not renewed when they were incarcerated, which are both useful factors as decision-making aids, when it comes to identifying the predictive signs and indicators that a subject is about to abandon treatment (or anticipating abandonment of treatment). |
