Au centre médical Marmottan s'est développée en consultation ambulatoire une pratique clinique de prescription-délivrance « de la main à la main » de médicaments. Elle concerne tout type de médicaments, les traitements psychotropes aussi bien que les traitements de substitution ou somatiques. Comme l'ensemble des soins au centre, cette pratique est anonyme et gratuite. Les traitements déconditionnés sont remis au comprimé près, au cours de l'entretien, par le médecin prescripteur ou un infirmier, pour des durées courtes, avec même la possibilité d'une délivrance quotidienne avec prise sur place si la situation le requiert. L'outil que constitue ce cadre de prescription et de délivrance de traitement permet non seulement un accès facilité au traitement pour les personnes les plus démunies, mais aussi un mode d'accompagnement psychosocial privilégié dont l'intérêt ne se limite pas à éviter le détournement et l'abus de psychotropes. Il aide à la construction très progressive d'une alliance thérapeutique entre le patient, le thérapeute et l'institution. (Résumé d'auteur.) ENGLISH : In the Marmottan medical centre, a clinical practice has been implemented in out-patient consultation that involves prescribing and directly dispensing medication. It concerns all types of medicines, including psychotropic drugs and substitute or somatic treatments. Like all care in this centre, this practice is anonymous and free of charge. The medicines are removed from their packages and a precise number of pills dispensed during an interview with the prescribing doctor or a nurse. This procedure functions on a short term basis, with the possibility of daily provision if necessary, or even the taking of pills on the spot if required. This way of prescribing and dispensing medication constitutes a tool which not only facilitates access to treatment for those in need, but also sets up a positive mode of psychosocial care whose ambitions are not limited to avoiding the trading and abuse of psychotropic drugs. It is contributing little by little to the construction of a therapeutic alliance between patient, therapist and the institution. (Author's abstract.) |
