L'étude porte sur une cohorte de 644 patients schizophrènes de la région de Stockholm ayant eu une conduite violente entre 1972 et 1986. 38 patients ayant répondu de 71 actes violents ont été retenus pour évaluer le rôle joué par l'alcool et la drogue. Parmi ces patients, 14 ont été retrouvés alcooliques et/ou drogués et 7 autres ont été supposés l'être. Ces chiffres sont plus élevés que ceux qui avaient été précédemment rapportés concernant les schizophrènes. La plupart des actes violents, de même nature que ceux commis par des individus non psychotiques, ont été perpétrés par des patients de longue date et non hospitalisés. L'étude met en évidence le rôle de l'alcool, de la drogue et de la désinsertion sociale dans le comportement violent des patients schizophrènes. Sur 24 actes de violence, 23 ont été commis par des schizophrènes alcooliques et/ou toxicomanes. ENGLISH: To assess the role of alcohol and drug abuse among schizophrenics with assaultive behaviour, we analysed case records of 38 patients who were responsible for a total of 71 violent offences between 1972 and 1986. They were identified from a population-based cohort of 644 individuals with schizophrenia hospitalized in Stockholm County during 1971. Fourteen of the 38 offenders abused alcohol and/or drugs and another 7 were probable abusers. These figures are higher than previously reported as regards abuse among schizophrenics. Most of the offences were performed by nonhospitalized patients who had been ill for many years. The nature of most of the violent incidents resembled that of offences committed by non-psychotic individuals. Our findings give further evidence for the role of abuse and social disintegration in assaultive behaviour among persons with schizophrenia. |
