FRANÇAIS : Essai randomisé en double aveugle, sur 18 patients, visant à définir les caractéristiques de la vasoconstriction coronarienne induite par la cocaïne ainsi qu'à évaluer ses liens avec les concentrations sanguines en cocaïne et en métabolites. L'observation du diamètre de l'artère coronaire, 30, 60, 90 minutes après administration de cocaïne intranasale, met en évidence une première diminution (à 30 mn), correspondant à un pic temporaire de la cocaïne dans le sang, suivie d'un retour à l'état de base (à 60 mn) puis d'une vasoconstriction récurrente en relation avec une augmentation des concentrations sanguines des métabolites de la cocaïne (benzoylecgonine et éthyl-méthyl ecgonine). Aucun changement n'étant observé dans le groupe témoin, les auteurs concluent que la cocaïne par voie intranasale est responsable de vasoconstrictions récurrentes qui peuvent être induites par ses métabolites. ENGLISH : To define the temporal characteristics of cocaine-induced coronary vasoconstriction in humans and to assess the relation between cocaine-induced coronary vasoconstriction and the blood concentration of cocaine and its main metabolites. Eighteen patients (16 men and 2 women, 37 to 65 years of age) having catheterization for evaluation of chest pain were studied. Cineangiographic examination of the left coronary artery and quantitation of the blood concentration of cocaine and its metabolites were done before and 30, 60, and 90 minutes after administration of intranasal saline or cocaine. In response to cocaine, proximal coronary arterial diameter decreased at 30 minutes. This change corresponded temporally to the peak blood concentration of cocaine. At 60 minutes, the cocaine concentration decreased and coronary artery diameter returned to baseline. At 90 minutes, all patients had recurrent vasoconstriction despite a further decrease in the blood cocaine concentration. This vasoconstriction corresponded temporally with an increasing blood concentration of cocaine's main metabolites, benzoylecgonine and ethyl methyl ecgonine. No changes were observed in the control group. In conclusion, intranasal cocaine causes recurrent coronary vasoconstriction, which may be due to its metabolites. (Extract from author's abstract) |
