L'usage conjoint d'héroïne et de cocaïne est étudié, en regard à d'autres pharmacodépendances, dans un échantillon de 1436 patients admis en cure de désintoxication en hopital psychiatrique durant la période d'octobre 1985 à octobre 1990. L'analyse porte sur des variables liées à l'histoire de la dépendance et lors de l'admission en traitement, le diagnostic de double dépendance est réalisé par analyse d'urine. Les résultats montrent une augmentation (de 17% à 32%) du nombre des héroïnomanes consommateurs de cocaïne, ainsi que des complications sociales et médicales dans cette population. Cette étude est complétée par une synthèse des investigations neuropharmacologiques précliniques antérieures, concernant la synergie des deux drogues sur les effets de renforcement liés aux opioides endogènes et aux systèmes dopaminergiques, et par un bilan des caractéristiques et conséquences cliniques de cet usage conjoint. ENGLISH : The amount of heroin and cocaine addicts with regard of another kinds of drug dependences has been studied together with data concerning the history of addiction, and the presence in the urine analysis of the cocaine on the day of hospital admission. This work has been realised in a sample of 1436 drug dependents admitted for treatment to Detoxication Unit of the Psychiatric Hospital in Madrid, between october 1985 and october 1990. In the results there was an increase in the diagnostic of the heroin and cocaine addiction (17% to 32%), and of the urine analysis proved positive for cocaine (17% to 38%). Also the cocaine and heroin addicts show more toxicologic and social complications than the remainder of addicts. In this study was revised the previous preclinic neuropharmacologic investigations, about the sinergistic action between the two drugs of abuse on the effects of reinforcement, which are caused by the endogenus opioid and dopaminergic systems, as well as the clinic characteristics and outcomes of the joint use.(Author's abstract) |
