FRANÇAIS : Cet article propose une revue de la littérature sur la relation entre dépendance et les états limites et un compte-rendu d'une étude exploratoire de cette relation. L'étude a été menée auprès d'un groupe de 25 toxicomanes fréquentant un CAD (Centre pour Alcool et Drogues), les résultats confirment l'hypothèse d'un lien existant entre dépendance et l'état limite. Quelques difficultés théoriques et méthodologiques sont abordées tel que la définition et l'opérationalisation du concept "état limite". Il parait important dans ce context de différentier les états limites "descriptifs" et "structurels", ainsi que les divers instruments diagnostiques correspondants. Une conclusion provisoire priviligerait pour le constat d'une pathologie d'état limite chez un patient très dépendant, un diagnostic structurel. L'article comporte également quelques remarques sur les indications et le traitement de toxicomanes ayant une structure "état limite". ENGLISH : This article gives an overview about the literature on the relationship between substance dependence and borderline-pathology. At the same time the authors present a pilot study that investigated a group of 25 clients who came to a Centre for Consultation for Alcohol and Drugs. The results confirmed the clinical experience that in a population of severe addicts there is more than normal chance to find a high percentage of borderlines. Some theoretical and methodological complications are discussed, for example the different borderline concepts that are used. The authors conclude that it is important to differentiate between descriptive and structural borderline-pathology (and the different diagnostic instruments that are developed in this context) and they give arguments that for diagnosing borderline personality disorder between (severe) addicts, Kernberg' structural interview is the instrument of choice. Moreover, some remarks are made about indication for treatment. (Authors'abstract) |
