FRANÇAIS : Dans les années 70-80,la recherche sur les risques d'une exposition prénatale aux opiacés avait mis en évidence les facteurs de risque biologiques, mais aussi sociaux, et la nécessité d'une approche multifactorielle. Toutefois, la recherche sur les risques liés à la cocaïne a pris le dessus, délaissant les modèles proposés auparavant. En présentant une synthèse de la littérature sur l'action des opiacés, l'auteur espère contribuer à l'élaboration d'un paradigme pour le futur. L'idée générale est celle d'une prise en compte, à la fois,des effets sur la mère et sur l'enfant. ENGLISH : By the mid-80's most reseachers recognized that not only biological but social and environmental risk factors must be considered to investigate perinatal and developmental outcomes associated with perinatal opiate exposure. However, prenatal cocaine exposure became a priority. Unfortunately, rather than employ the model emerging from the experience of opiate investigations, initial research directed at identifying effects of cocaine exposure often employed the same type of bi-variate approach,naive to the cumulative risks concomittant to maternal substance abuse. It is within this context that an overwiew of existing research on prenatal opiate exposure will be presented with the hope that the limitations of the past can be used to construct a useful paradigm for the future. Efforts need to be conducted within the context of maternal addiction and concomittant effects on both mother and infant. (Extract from author's abstract) |
