Cet article concerne les effets cliniques d'abus et d'arrêt de cocaïne chez des schizophrènes à deux moments : celui où les patients sont admis en urgence psychiatrique, et après 4 semaines d'hospitalisation. L'étude porte sur 15 cocaïnomanes et 22 schizophrènes abstinents. Les schizophrènes cocaïnomanes présentent moins de symptômes péjoratifs et plus de dépression et d'anxiété à leur admission que leurs homologues abstinents. Au deuxième test, on n'a relevé aucune différence de groupe concernant les symptômes péjoratifs ou dépressifs des patients. Les symptômes positifs sont aussi intenses dans les deux tests. La différence significative des symptômes péjoratifs et dépressifs au moment de l'admission est dûe à l'effet neurobiologique de la cocaïne. Le rôle des psychostimulants chez des patients schizophrènes est discuté. ENGLISH : This report concerns the clinical effects of cocaïne abuse and cessation in schizophrenic patients at two times : when patients presented to the psychiatric emergency service and again after 4 weeks of hospitalization. The subjects were 15 cocaine-abusing and 22 cocaine-abstaining schizophrenic patients. Cocaine-abusing schizophrenic patients showed fewer negative signs and more anxiety/depression at the hospital-admission assessment than their nonabusing counterparts. At retest, no group differences were detected in patients' negative signs or mood symptoms. Severity of positive symptoms was equal at both testing sessions. The significant difference in negative signs and mood symptoms at admission assessment was attributed to the neurobiological impact of cocaine. The role of psychostimulants in schizophrenic patients is discussed. (From the authors'abstract) |
