FRANÇAIS : L'étude des anticorps anticardiolipine (aCL) isotype IgG sur 65 patients toxicomanes par voie parentérale (ADVP) a permis l'analyse de sa possible relation avec le VIH. Dans cette étude, 37 patients étaient infectés par le VIH, dont 19 (51%) remplissaient les critères diagnostiques du SIDA. Parmi ces 37 patients au VIH positif, 32 (86%) étaient IgG-aCL positifs et 14 (50%) des 28 patients au VIH négatif étaient IgG-aCL positifs. L'étude met en évidence l'absence de relation entre les anticorps anticardiolipine et la thrombopénie. Aucun patient ne présente de complications thrombo-emboliques. On a observé des niveaux très élevés de IgG-aCL (>80 GPL) sur 3 patients ayant le SIDA et des endocardites infectieuses. En conclusion, du fait de la non spécificité de la présence des aCL chez les patients toxicomanes intraveineux, on considère que sa détermination est d'une faible utilité dans la pratique clinique journalière. ENGLISH : A prospective study was performed in 60 IVDA patients previously untreated and distributed into three groups : 20 seronegative for the human immunodeficiency virus (HIV), 20 patients in a phase of generalized adenopathic infection (GAI) and 20 patients with AIDS. Cytohistologic examination of the bone marrow aspirates showed plasmocytosis and eosinophylia in all the groups. Selective changes were seen in the red series in 20% of the HIV negative patients and in 25% of the GAI group. Pathologic structure showed granulomatous lesions of tuberculous etiology in 30% of the patients with AIDS while in the HIV negative and GAI groups there were found in 10% and 5%, respectively. A greater presence of fibrosis and bone marrow hypoplasia was also found in the group with AIDS, than in the other two groups. (From the authors' abstract) |
