FRANÇAIS : Les programmes de traitement à la méthadone pour les toxicomanes usagers d'opiacés ont été introduits à Hambourg en été 1988. Le nombre de patients en substitution y est en constante augmentation. Depuis 1991 une recherche scientifique de suivi est effectuée. L'article en présente certains résultats qui prouvent tous des effets positifs de ces traitements. Le taux de rétention est de 91.3%, ce qui est très élevé. La dose de méthadone (10 ml de levo methadone en général) dépend principalement de la dose d'héroine prise avant par le patient. La consommation additionnelle d'héroine de cocaine ou de cannabis n'a aucune influence sur le dosage de la méthadone. L'évolution est favorable en terme de santé, de commencement de réinsertion sociale et de diminution, voire d'abandon de l'usage d'un autre produit. En outre, le contexte de la substitution permet une thérapie psychosociale intensive. Thérapeutes et patients considèrent ces thérapies additionnelles efficaces. ENGLISH : Methadone treatment for opiate addicts as a structured form of therapy has been introduced in Hamburg in the summer of 1988. The number of patients in substitution has been increasing ever since. Since 1991 scientific follow-up research has been done. The following presents a number of selected results, which show the overall positive effects of methadone therapy. The retention rate is 91.3%, which is extremely high. The dose of methadone (10 ml levo-methadone in general) depends primarily on the dose of heroin a patient was using before the beginning of substitution. The additional use of heroin, cocaine or cannabis does not have any influence on the dosage of methadone. On the whole, we can see positive developments : the patients' health improved substantially, social reintegration started, the additional use of drugs was given up or decreased. Furthermore the conditions of substitution allow an intensive psychoand sociotherapy. Both therapists and patients consider such additional therapies effective. (from authors' abstract) |
