FRANÇAIS : Les mécanismes cellulaires de base de la cocaïne, responsables des effets cardiovasculaires, résident en une potentialisation des effets des catécholamines due à une inhibition de l'action anesthésique présynaptique locale par blocage des canaux sodiques. Aprés le rappel de ces mécanismes, l'article fait une synthèse des principales maladies cardiovasculaires dues à la cocaïne : angine de poitrine et infarctus du myocarde, arythmie et fibrilation ventriculaire, myocardite et cardiomyopathie et enfin, les endocardites bactériennes liées à l'usage de cocaïne par voie intraveineuse, et tente d'apporter une explication pharmacologique à chacune de ces pathologies. ENGLISH : Cocaine is accompanied by a high risk of serious adverse effects involving the cardiovascular system. The basic cellular mechanisms of cocaine consist in a potentiation of catecholamine effects by inhibition of the presynaptic uptake carrier local and in anesthetic effects by the block of sodium-channels. Acute ischemic events can be induced by cocaine through coronary spasms. Procoagulant properties of cocaine may, moreover, favor coronary thrombosis formation and the development of myocardial infarction. Conduction velocity impairments and inhomogeneous repolarization phenomena may form a substrate for re-entrant circuits inducing ventricular fibrillation. Cocaine abuse may also cause degenerative and inflammatory alterations of the myocardium. Moreover, persons using cocaine intravenously seem to be particularly endangered by bacterial endocarditis compared to the users of other intravenous drugs. (Extract from authors' abstract) |
