En Europe, la toxicomanie est devenue un grave problème de santé publique avec une augmentation de la délinquance, de la prostitution, des surdosages et du Sida. Les programmes de TMM associés à un contrat thérapeutique et un soutien psychosocial, constituent l'approche la plus efficace pour limiter les risques de dépendance, diminuer la consommation d'héroïne et maintenir les toxicomanes à un niveau psychologique et social correct. L'auteur présente ici les résultats d'une évaluation concernant 280 toxicomanes inscrits dans trois programmes de maintenance à la méthadone de la Fondation Phénix de Genève. Les statistiques indiquent qu'un bon nombre de patients rechutent dans l'année qui suit l'arrêt du traitement à la méthadone. Pour limiter les rechutes, il faut attendre que les patients soient parfaitement stabilisés d'un point de vue médical, psychologique, social, affectif et toxicologique. ENGLISH : In most of Europe, drug addiction has become one of the basic public health issues with a steep rise in delinquency, prostitution, the sequels of overdoses and the transmission of Aids. Methadone treatment programs accompanied by a therapeutic contract and psycho-social support constitutes the most effective approach for limiting the risks of addiction, decreasing the frequency of heroin use and maintaining addicts at a good psycho-social level of stability. Here, the author presents the results of an evaluation concerning 280 drug addicts in three methadone treatment programs of the Phenix Foundation in Geneva. Statistics indicate that a great majority of patients relapse within the year following the withdrawal from methadone. To limit these relapses it is necessary to wait until patients are perfectly stabilized from the medical, psychological, social, affective and toxicological point of view.
