FRANÇAIS : La méthode de recherche "Zelen" est une forme modifiée de la méthode traditionnelle de randomisation. Il s'agit d'une prérandomisation, suivie d'une information et d'une demande de consentement aux personnes appartenant au groupe expérimental. L'applicabilité de cette méthode est controversée, mais offre dans certains cas des avantages par rapport à la méthode traditionnelle. Notamment dans des cas où l'expérience est très attirante pour les participants. Dans ce cas, en effet, la méthode traditionnelle permet rarement d'obtenir des résultats valides à cause d'un important taux d'abandon dans le groupe de contrôle, dont les participants se sentent frustrés de ne pas recevoir le "meilleur" traitement. Dans la méthode de Zelen, les participants du groupe de contrôle ne savent pas qu'ils sont impliqués dans une recherche et ils reçoivent leur traitement habituel. Les auteurs décrivent quelques autres conditions à remplir avant d'appliquer la méthode Zelen et développent la comparaison entre méthode traditionnelle et Zelen. Les propose sont illustrés à partir d'un projet de recherche sur la distribution randomisée d'héroïne ENGLISH : The Zelen design is a modification of the traditional randomized design, also called 'randomized consent design' or 'prerandomization'. Contrary to the traditional procedure, in the Zelen design eligible patients are randomized before informed consent is obtained from the patients in the experimental group only. The applicability of this design is controversial. However, there are circumstances in which it is preferable to the traditional design. One of the conditions under which the Zelen design might be used is that the intervention must be highly attractive for the eligible patients. In the article, conditions for the use of the Zelen design are discussed in more detail as well as advantages and disadvantages of both designs.
