FRANÇAIS : De nombreux efforts sont faits actuellement pour trouver de nouveaux traitements médicamenteux de la toxicomanie, et l'auteur souligne ici l'importance d'une structure conceptuelle pour mieux appréhender toutes les dimensions de ce problème complexe. La première dimension concerne la classification pharmacologique; la seconde concerne l'intervention thérapeutique pharmacologique (prévenir le début de la toxicomanie, contrer les effets toxiques, permettre le sevrage et prévenir les rechutes). Le statut de la drogue constitue la troisième dimension et détermine le temps, l'effort et l'argent nécessaires à son traitement. Enfin la dernière dimension comprend les études cliniques visant à optimiser l'efficacité de la recherche en matière de nouveaux traitements. ENGLISH : There is currently renewed interest in the systematic and aggressive development of new medications to aid the treatment of drug abuse, and a conceptual framework is useful in considering the many dimensions of this complex problem. The first dimension is the particular drug of abuse, where a pharmacological classification is helpful. The point of potential pharmacotherapeutic intervention is a second dimension, where preventing initiation of drug abuse, antagonising toxic effects, making the diagnosis, treating withdrawal, and preventing relapse are the principal areas of interest. The status of a drug is an important determinant of the time, effort and money needed to develop it for treating drug abuse. Lastly, for optimal efficiency in the development of such medications, preclinical and clinical studies should be closely integrated, so that only the amount of preclinical testing needed for the next phase of clinical trials is undertaken before initiating that phase.
