Les programmes de maintenance pour héroïnomanes au moyen d'agonistes opiacés supposent l'administration continue de médicaments de substitution de l'héroïne par voie orale, sous contrôle médical. Parmi les médicaments agonistes des récepteurs opioïdes autorisés dans ce type de programmes, la méthadone est le plus utilisé. Or, le principal inconvénient que présente la méthadone est qu'elle nécessite une administration quotidienne, ce qui n'est pas le cas avec le LAAM dont la durée de demi-vie pourrait permettre une administration toutes les 48h. à 72h. L'héroïne elle-même est utilisée également dans certains programmes de maintenance dans le cadre d'une expérience pilote en Suisse, avec pour objectif principal d'atteindre des héroïnomanes peu motivés et de les enrôler dans un programme méthadone. ENGLISH : The heroin addict maintenance programmes with opioid agonists involve the continuous oral administration to these patients of a heroin-substitute drug under medical control with no clear goal of disintoxication. Amongst the opioid receptor drugs authorized in these maintenance programmes methadone continues to be the most widely used drug at the present time. Two types of programme, with a high and low therapeutic requirement threshold, are differentiated on the basis of the objectives foreseen. The main drawback about methadone is that it has to be administered daily, a drawback that could be avoided with levo-alpha-acetyl- methadol (LAAM), due to its longer half-life, which could allow it to be administered every 48-72 h. This drug has recently been authorized for use in these programmes and is about to come on the market in Spain. Heroin itself is being used in maintenance programmes in a pilot experiment that is taking place in Switzerland with the idea of recruiting heroin addicts who are sufficiently motivated to start a methadone programme.
