FRANÇAIS : Les circonstances socio-démographiques des décès liés à l'héroïne, ainsi que les résultats toxicologiques ressortant de l'autopsie de 152 cas d'overdose (New South Wales, 1992) sont décrits. Il semble que la cause majeure de décès ne soit pas liée à la pureté de l'héroïne mais à des polyconsommations (alcool, benzodiazépines). La plupart des overdoses a lieu dans l'environnement du domicile et à proximité d'autres personnes. Seulement 14% des décès se produisent immédiatement (60% surviennent dans un délai d'au moins une heure) mais dans 90% des cas les secours ne sont pas appelés à temps. Il paraît potentiellement possible d'éviter ces décès, en prévenant les usagers des risques liés aux associations de produits, en les formant aux premiers secours, en leur montrant la nécessité de faire appel aux services d'urgence. ENGLISH : The aims of the study were to describe the sociodemography, circumstances of death and toxicological findings at autopsy of the 152 cases of heroin related deaths (HRDs) in New South Wales in 1992. The findings suggest that heroin related deaths may be due to causes other than increased purity of heroin. In the majority of cases depressant drugs in addition to morphine were detected at autopsy. These were ususally alcohol and benzodiazépines, indicating that polydrug use may be a major contributory cause of death. Research into the circumstances of HRDs showed that most deaths occurred in a home environment, in the near vicinity of other persons, and that in 90% of cases calls to ambulance service were delayed or were not made at all. These findings indicate that fatal heroin overdose is potentially preventable and that a number of strategies could be developped to contribute to the reduction of these deaths. For example there is a need to educate heroin users about the risks of co-administration of alcohol and other depressant drugs with heroin, train heroin users in first aid techniques, the use of heroin in the company of others, and to call for intervention sooner. |
