FRANÇAIS : A partir du traitement psychothérapeutique psychodynamique d'une patiente cocaïnomane de 23 ans, l'auteur décrit les concepts théoriques et les méthodes cliniques qu'il a appliqués. Une approche s'inspirant des travaux de Winnicott et de Malan lui permet de décrire comment la psychothérapie psychodynamique peut produire des effets facilitant le changement dans une addiction en traitement ambulatoire. Les travaux de Winnicott fournissent le contexte du traitement tandis que le modèle de Malan la structure du processus thérapeutique. Dans le contexte d'un environnement d'attente, les liens entre traumatisme passé et consommation présente de drogue comme défense sont examinés, tout autant que l'interprétation des rêves, les expériences émotionnelles rectificatives et l'utilisation en plus de programme en 12 étapes dans le processus thérapeutique. ENGLISH : Using the psychodynamic psychotherapeutic treatment of a 23 year old female cocaine-addicted client as an example, the author describes the theoretical concepts and clinical methods he has found useful in this context. Using an approach that draws on the work of Winnicott and Malan, the author describes how psychodynamic psychotherapy can be effective in facilitating change in an addiction in an outpatient setting. Winnicott's work provides the context for the treatment while Malan's model provides a structure for the therapeutic process. Within the context of a holding environment, links between past trauma and current drug use as a defence are explored. Also discussed are the use of dream material, the facilitation of the corrective emotional experience, and the complementary use of 12-step programs in the therapeutic process. (Author's abstract) |
