FRANÇAIS : Les hépatites virales, notamment celles à virus B et C, sont très répandues parmi les alcooliques et les usagers de drogues par voie injectable au Royaume-Uni. Pour la prévention de ces infections, un dépistage des hépatites B et C pourrait être pratiqué dans les services de traitement, suivi d'une vaccination contre l'hépatite B pour les patients non immunisés ou d'examens fonctionnels du foie pour les patients présentant des anticorps. Le traitement à l'interféron est efficace dans de nombreux cas, mais il est cher et de prescription délicate. Actuellement, les interventions sont centrées sur les changements des comportements à risques et sur l'identification des usagers qui ont le plus besoin d'un traitement anti-viral. Plusieurs nouveaux anti-viraux en cours d'évaluation clinique et une prospection pour la conception de vaccins laissent espérer des progrès notables dans le traitement des hépatites chroniques dans les années à venir. ENGLISH : Viral hepatitis, particularly that due to HBV or HVC, is a significant problem in the UK population of IDVUs and alcool misusers. Patients attending drug rehabilitation clinics should be screened for HBV and HCV infection. All those found to be HBcAb negative should be vaccinated against HBV. Those found to be HBsAg or HCV antibody positive should have their liver function tests checked and be referred to a specialist liver clinic for further evaluation. Interferon treatment of HBV or HCV infection is effective in a proportion cases but it is an expensive agent that requires careful monitoring. Current managements regimes are centred around the modification of risk behaviour and the identification of those most in need of anti-viral treatment. Several new anti-viral agents are currently undergoing clinical evaluation and this together with the prospect of vaccines designed to clear an established infection means that the next few years are likely to see some major advances in the treatment of chronic viral hepatitis.
