FRANÇAIS : Le problème de la dépendance aux benzodiazépines (BZDs) est aggravé par la dépendance iatrogénique physiologique et par un usage associé à d'autres produits (polytoxicomanie). Lors du sevrage, un syndrome physiologique peut survenir avec rebond et récurrence. Les auteurs décrivent le syndrome de sevrage ainsi que son traitement à l'aide d'anticonvulsivants, la carbamazépine et le valproate. Ces deux médicaments sont efficaces, particulièrement la carbazépine. Cependant, la carbamazépine présente plus d'effets secondaires que le valporate et que les nouveaux anticonvulsivants tel le gabapentin. En revanche, le valporate semble avoir l'avantage de pouvoir traiter à la fois la dépendance aux benzodiazépines et les symptômes de rebond. En conclusion, les auteurs suggèrent qu'il existe des différences importantes entre les patients dépendant des BZDs dans le cadre d'une polytoxicomanie et ceux dépendant suite à un usage thérapeutique. ENGLISH : The problem of dependence on benzodiazepines has been aggravated by iatrogenic pbysiologic dependence on these medications and by polysubstance abusing patients using them in addition to other agents. Patients withdrawing from these agents may experience physiologic withdrawal, rebound, and recurrence. The authors review the typical syndrome associated with withdrawal from benzodiazepines and discuss treatment of benzodiazepine withdrawal using carbamazepine and valproate. The evidence is good that anticonvulsants, particularly carbamazepine, help alleviate the symptoms of BZD withdrawal. Carbamazepine, has more side effects than valproate and other new anticonvulsants, specifically gabapentin. Valproate may potentially treat both BZD dependence and symptom recurrence/rebound. Finally, there may be an important difference between patients dependent on BZDs as part of their polydrug addiction and those dependent on BZDs for therapeutic purposes. (From authors' abstract) |
