Les médicaments opioïdes tiennent un rôle important dans le traitement de la douleur aigüe, de la douleur due au cancer et parfois pour des douleurs intraitables d'origine non cancéreuses. Cependant certains individus peuvent développer une dépendance suite à un usage prolongé d'opioïdes. Cet article présente les recommandations de l'American Society of Addiction Medecine (ASAM) pour l'usage d'opioïdes comme traitement de la douleur portant sur la dépendance physique, la tolérance et l'addiction. L'ASAM précise qu'il peut s'agir d'une addiction quand il existe un des trois effets suivants : des effets contraires associés à l'usage d'opioïdes, une perte de contrôle liée à l'usage d'opioïdes, une obsession pour obtenir des opioïdes malgré le choix d'un analgésique approprié. ENGLISH: Opioid medications have an important role in the treatment of acute pain and cancer-related pain, and they are sometimes helpful as a component of the management of intractable pain of non-cancer origin. However, a number of issues of clinical concern may arise in the course of opioid therapy of pain. Physical dependence on opioids may occur with prolonged therapeutic use, and tolerance to the analgesic effects of opioids may occur under certain circumstances. Some individuals may develop addiction in association with the prolonged use of opioids. The clinical implications and appropriate management of physical dependence, tolerance and addiction differ. It is therefore important that clear definitions be established to facilitate identification and appropriate management of these occurrences. The standard DSM IV criteria for diagnosis of psychoactive substance use disorder cannot be used reliably to diagnose addiction in the presence of opioids prescribed for the treatment of pain. Many of the DSM IV criteria defining addiction refer either to physical dependency or tolerance which are physiological and expected with the long-term opioid use (criteria 1 and 2) or to other occurrences which may be normal and expected in the course of opioid therapy of chronic pain (criteria 3 and 4). If DSM IV criteria are considered in diagnosing addiction in the context of pain treatment, only those criteria which reflect addictive behaviors (criteria 5, 6, and 7), rather than the physiologic phenomena of physical dependency and tolerance, should be used in formulating the diagnosis.
