FRANÇAIS : Dans le but d'évaluer l'efficacité du disulfiram et de trois formes de psychothérapie pour personnes dépendantes de la cocaïne et de l'alcool, 122 usagers d'un centre de traitement ont été répartis entre 5 traitements pendant 12 semaines : le Cognitive Behavioral Treatment (CBT) avec et sans disulfiram, le Twelve Step Facilitation (TSF) avec et sans disulfiram, et le Clinical Management avec disulfiram. Le traitement au disulfiram apporte un meilleur maintien en traitement et une plus longue abstinence d'alcool et de cocaïne. Les deux psychothérapies actives, CBT et TSF, semblent liées à une diminution progressive de la consommation de cocaïne. L'usage d'alcool et de cocaïne est fortement lié pendant le traitement notamment ceux avec disulfiram. Le disulfiram associé à une psychothérapie ambulatoire apparaît prometteur dans le traitement des troubles associés. Cette étude permet de souligner l'importance de l'alcoolisme chez les cocaïnomanes désirant un traitement et l'importance d'associer la psychothérapie à la pharmacothérapie dans le traitement de la toxicomanie. ENGLISH : To evaluate disulfiram (D) and 3 forms of psychotherapy for cocaine addicts with concurrent alcohol abuse, 122 cocaine/alcohol abusers were allocated in 5 treatments : cognitive behavioral treatment (CBT) with and without D, Twelve Step facilitation (TSF) with and without D, clinical management plus D. Disulfiram treatment was associated with significantly better retention in treatment and longer duration of abstinence from alcohol and cocaine use. The 2 active psychotherapies (CBT, TSF) were associated with reduced cocaine use over time. Cocaine and alcohol use were strongly related throughout treatment, particularly for subjects treated with disulfiram. For the large proportion of cocaine abusers who also abuse alcohol, disulfiram combined with outpatient psychotheray may be a promising treatment strategy. This study underlines the significance of alcohol use among treatment-seeking cocaine abusers, the promise of the strategy of treating co-morbid disorders among drug addicts, and the importance of combining psychotherapy and pharmacotherapy in the treatment of drug use disorders.
