FRANÇAIS : Cet article décrit une étude avec 91 héroïnomanes (selon les critères du DSM-IV) désintoxiqués dans une Unité Ambulatoire MédicoChirurgicale à l'hôpital de Santiago Apostol (Espagne) de septembre 1994 à février 1996. Le syndrome d'abstinence aux opiacés a été traité avec la naltrexone (50 mg) et la naloxone (0.5 mg) après avoir réalisé un pré-traitement avec clonidine (0.4 mg), midazolam (48 mg) et ondasetron (6.6 mg). En 12 heures tous les patients ont été désintoxiqués. 80 patients on pu sortir de l'hôpital et 77 patients ont commencé leur traitement le lendemain. La méthode employée réduit les symptômes de sevrage comme pour d'autres méthodes, atteint de meilleur taux de retour que les autres et est économiquement intéressante. La stabilisation préalable des patients avec des opiacés antagonistes n'est pas associé à une symptomatologie particulière du sevrage pendant la désintoxication. (D'après le résumé des auteurs) ENGLISH : 91 heroin dependents (according to DSM-IV) detoxificated in a medical-surgical out-patient department in Santiago Aspostol Hospital (Spain) between September 1994-February 1996 have been analysed. The opiate withdrawal symptoms have been treated with naltrexone (50 mg) and naloxone (0.5 mg), after a pre-treatment with clonidine (0.4 mg), midazolam (48 mg) and ondasetron (6.6 mg). In 12 hours, all the patients have been detoxificated. 80 patients have been discharged from hospital, and 77 patients started their respective treatment. The system used reduces withdrawal symptoms, as well as other pharmacological systems developped so far and reaches greatest rates of return than other methods used and it is not so expensive. The previous stabilization of patients with opiate antagonists is not associated to a smaller withdrawal symptomatology during the detoxification. (From authors'abstract) |
